• Mental Health Screening (You must opt your student in, if you wish to have this screening completed)

10th Grade:

• Height
• Weight 
• Vision

11th Grade:

• Height
• Weight
• Vision
• Hearing
• Mental Health Screening
• Physical Exam (Sports and Drivers physicals count, let us know if your student had either of these completed and send a copy to the nurse's office)

12th Grade:

• Height
• Weight
• Vision
• Meningococcal Booster
